?Participant releases, discharges, and absolves any and all liability for Enso Healing Therapy & Lilly's Internal Light, LLC, including any training/CEU courses, consulting services, vendor events, fitness, dance, and/or wellness instructors (parties, agents, and/or affiliates), from any injury/accident during any fitness, dance, or wellness class. *Classes, trainings, and any other service or product are NOT to be a replacement for therapy or coaching of any kind. *CEUs, other training Courses, and all the above are for informational, developmental, and educational purposes only. *By attending you agree to being recorded and will not be compensated in any form. (Please note not all services or products are recorded, please view class details for more information) ?This release shall be binding upon participant?s heirs, executors, administrators and assigns. Participant assumes the risk of any and all accidents or injuries of any kind. In the event participant is injured, participant agrees to assume any financial obligation. ?Enso Healing Therapy assumes no responsibility for any medical expenses, injury, or damage suffered by Participant in connection with the use of any facilities or services in connection with the activity. *I understand the following with respect to in-person sessions during the Covid-19 pandemic: a. I understand that Covid-19 is extremely contagious and is spread primarily by person-to person contact. b. I understand that my instructor has adopted reasonable preventative measures intended to reduce the spread of Covid-19, but there is still a possibility of transmission as a result of attending in-person classes. c. I understand that federal and state laws typically authorize public health departments to collect student information to prevent or control disease and for related public health needs. d. I understand that my instructor may be required to report Covid-19 related student information to public health departments, HHS, or the CDC. For example, if anyone who has been in my instructor?s office tests positive for Covid-19, disclosure may be necessary for contact tracing or other data collection needs. If reporting is required, only the minimum necessary information will be disclosed. 2. I agree to the following with respect to in-person sessions/ classes during the Covid-19 pandemic: a. I will comply with safety precautions to limit the spread of Covid-19, as directed by my instructor. b. I will notify my instructor as soon as possible before my appointment if I have symptoms of Covid-19 or have been exposed to certain risk factors as directed by my instructor. If this happens, I will cancel my appointment unless my instructor directs me to come in. I knowingly and willingly consent to have in-person sessions during the Covid-19 pandemic, and I acknowledge the health risk of Covid-19 during this pandemic. I have read the information provided above and discussed it with my instructor, and all of my questions have been answered to my satisfaction ?Participant has been cleared by his/her medical doctor to participate in fitness, dance, and wellness classes. ?Emails are not shared and only used for correspondence regarding class (ie, cancelations, rescheduling, promotions, etc). * Participant attest to being at least 18 years of age * No Video recording of the class is permitted.
